Our ideal candidate is a self-starter, capable of supporting a team towards a common goal of providing exemplary palliative care. We're searching for someone with a minimum of five years of post-graduate experience in palliative care or related specialties, with prior leadership experience being highly desirable.
As the Urban Team Coordinator, you'll collaborate closely with our Inpatient Unit Team Coordinator and Rural Team Coordinator to ensure equitable access to hospice services across Otago. Additionally, you'll be our liaison with the wider healthcare community in urban Dunedin, forging partnerships to enhance patient care and support.
Applicants must hold a current annual practicing certificate, be eligible to work in New Zealand, and possess a clean driving license. This is a full-time, permanent position based in Dunedin.

We provide care for people with terminal illness, enabling them to live their lives as fully as possible. We work in partnership with our community, offering education, support, care and advice to family, whānau, carers and health and social service professionals, through a patient’s illness and following death.

Purpose of the position
Effective leadership and coordination of OCH community nursing and family support teams. Though:
- Promotion of excellence in palliative care service provision.
- Provision of effective clinical leadership
- Effective day to day management of allocated clinical team.
- Maintaining staffing levels and skill mix to meet required need.
- Ensure team adhere to organisational policies and procedures.
- Develop and maintain collaborative relationships both internally and externally, including regular engagement with stakeholders.

Specific areas of responsibility (criteria for measurement)
1. Management of the (Urban/Rural) clinical team
- Provide clinical leadership for the community team.
- Provide day-to-day management to the community team.
- Ensure the team is working in accordance with OCH expectations, to meet contractual obligations and expected level of service provision.
- Mentor, support, and develop clinical staff knowledge and skills.
- Identify, participate in and where appropriate lead; selection, orientation, appraisals and performance management processes.
- Ensure clinical staff work within their legislative boundaries, adhere to organisational policies and procedures and conduct their activities in a safe and ethical manner
- Maintain regular ongoing contact with all direct reports.
- Manage resources, including procurement, within the budget.
- Ensure the appropriate level of staff, skill, and experience is always available to meet the needs of the service, to ensure service delivery is always maintained, at the highest standard.
- Effectively identify, minimise, manage and monitor risk associated with service delivery elevating concerns as appropriate.
- Monitor performance, capability and needs of the clinical team. Identifying issues/concerns/needs and effectively addressing in line with OCH processes.
- Facilitate effective, high functioning clinical meetings.
- Work collaboratively with the other team coordinators, education team, clinical administration, SMT, and others within the organisation.
2. Clinical oversight of patients/whānau
- Maintains clinical oversight of patients/whānau, on the service.
- Be available to provide clinical support for all members of the team
- Allocation of patients to the clinical team, in accordance with need and clinical availability.
- Honour Tikanga Māori and our commitment to Te Tiriti o Waitangi maintaining an effective multi-cultural perspective, providing culturally appropriate care and culturally safe service.
- Avoid imposing prejudice on others and provide advocacy and guidance when prejudice is apparent within the team
- Monitor, develop and ensure compliance to expected standards within the organisation
3. Stakeholder Liaison
- Actively develop professional relationships with stakeholders, in order to promote collaboration, promoting the Hospice philosophy, vision and values through modelling and liaison with community and hospital health professionals
- Develop robust communication with associated services
- Participate in short and long term organisational planning
- Participate in local, regional or national collaborative initiatives
4. Service Development
- Actively involved in quality initiatives and projects that will advance service development within the organisation.
- Take a lead role in quality project work and project oversight within team
- Identify potential areas for service development within clinical service provision
- Support other clinical staff development though mentorship within service development projects
5. Education and training
- Participation in the education development team
- In collaboration with education, monitor and ensure ongoing professional development for team
- Develop processes in conjunction with Education Coordinator to ensure currency of staff professional development reflective of a specialist service.
- Participate in the planning, orientation and training for team and students in conjunction with Education Coordinator
6. Maintain Compliance
- Practice within professional standards, code of conduct, professional ethics and scope of practice.
- Is familiar with, promote and ensure compliance with hospice quality, health and safety and infection control policies.
- Maintain an active role in the hospice quality improvement programme which includes monitoring and promoting adherence to the Health and Disability Sector Standards and Hospice New Zealand Standards.
- Maintain own professional development relevant to the position, updating own clinical competencies, knowledge and skills as required.
- Work with all staff to ensure the clinical and physical environment is continually monitored for potential hazards and once identified these are managed.
- Encourage incident reporting and be involved/lead where appropriate implementation of remedial action and monitoring staff compliance
- Ensure professional standards, code of conduct, professional ethics, and scope of practice, and provide culturally appropriate care, are maintained at all times
- Understands and ensures compliance with OCH policies and procedures is upheld
- Promote Hospice New Zealand standards and quality initiatives
- Participation in audit processes
- Contribute, as requested, to the development of reports, policy and other relevant documentation
